29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: 83 Burlington St, Woburn, MA On about October 17, 2018, employees were exposed to electrical shock hazards while plugging and unplugging an extension cord into a receptacle that had the outer casing pulled away from the plug exposing the conductor wires.
