29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments: A) Autoplas Division, Windsor Mold Group, located at 560 Goodrich Road, Bellevue, OH 44811: On or about August 20, 2018, the employer did not ensure that employees were adequately trained in and familiar with electrical safety-related work practices such as, but not limited to, the use of special precautionary techniques and personal protective equipment. Maintenance employees were exposed to electrical hazards such as electrocution, electrical shock, and arc flash/blast when conducting activities such as, but not limited to, troubleshooting and checking part number for the disconnect on the electrical panel for the hydraulic 1000 Ton Demag Press # 13 (Model: Titan 900; Serial # 0508). B) Autoplas Division, Windsor Mold Group, located at 560 Goodrich Road, Bellevue, OH 44811: On or about September 11, 2018, the employer did not ensure that employees were adequately trained in and familiar with electrical safety-related work practices such as, but not limited to, the use of special precautionary techniques and personal protective equipment. Maintenance employees were exposed to electrical hazards, such as electrocution, electrical shock, and arc flash/blast when conducting activities such as, but not limited to, troubleshooting, cleaning, and replacing heater bands on the hydraulic 1000 Ton Demag Press # 13 (Model: Titan 900; Serial # 0508).
