29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) The employer did not ensure that lockout or tagout devices are affixed to each isolating device by authorized employees. This violation occurred on or about May 9, 2018, when lockout and/or tagout devices were not utilized by at least one (1) authorized employee clearing jam to the Baler machine.
