29 CFR 1910.217(e)(1)(i)(A): The employer did not conduct periodic and regular inspections of each power press to ensure that all of its parts, auxiliary equipment, and safeguards, including the clutch/brake mechanism, anti-repeat feature, and single-stroke mechanism, are in a safe operating condition and adjustment; On or about April 17, 2018, at the facility complex located at 6625 W. Wilshire Blvd in Oklahoma City, OK, the employer did not establish and implement a program ensuring periodic and regular inspections were conducted and documented for all power presses at the facility, exposing employees to amputation, caught-in and struck-by hazards.
