29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: a) At the shredder machine: On or about April 4, 2018, and at times prior thereto, employees were exposed to fire hazards in that a charged portable fire extinguisher was positioned on the ground as employees were expected to engage it for incipient fires in the area. b) At the vehicle refueling area: On or about April 4, 2018, and at times prior thereto, employees were exposed to fire hazards in that a discharged portable fire extinguisher was positioned on a barrier beam as employees were expected to engage it for incipient fires in the area. c) In the shredder motor room: On or about April 4, 2018, and at times prior thereto, employees were exposed to fire hazards in that a portable fire extinguisher was positioned on the floor as employees were expected to engage it for incipient fires in the area.
