29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: (a) Maintenance Area: On January 3, 2018 and times prior, an employee operated a Porter Cable abrasive bench grinder (serial #076349) that had an opening of 1 inch between the grinding wheel and the peripheral guard. The abrasive bench grinder was not equipped with a tool rest.
