29 CFR 1910.146(d)(3)(iii): The employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including, but not limited to isolating the permit space. a. Buckeye Ready-Mix, LLC located in Marion, Ohio: On or about February 28, 2018, the employer did not ensure the aggregate conveyor located above conjoined aggregate bins #2 and #5 were isolated to prevent an engulfment hazard for employees performing oxy-acetylene torching and welding inside of the bin below. b.
