29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): The employer is failing to protect employees from electrical shock hazards. This was most recently documented on 06/07/17 at the establishment located at 515 S. 4th Street in Osage City, Kansas. a) Employees in the production area were exposed to electrical shock hazards while operating the eDart system used with the #105 Cincinnati Milacron injection mold press machine. The eDart system was powered by an extension drop cord with a missing ground pin. b) Employees in the Maintenance Shop area were exposed to electrical shock hazards while operating the horizontal band saw machine. The cord supplying power was being used with damaged exposed wiring.
