29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) North side of entrance overhang: Employee without fall protection working approximately 18-20 feet without any fall protection was working from an inadequate wood scaffold, fabricated from leftover scrap lumber, on or about 02/24/2017; b) North side of entrance overhang: Employee without any fall protection approximately 18-20 feet above ground level, without any type of fall protection, was standing on top of an unsecured sheet of OSB resting on the forks of the Model B-66 Pettibone Telehandler while removing construction debris from the roof of the overhang, on or about 02/24/2017. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
