29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: The employer is failing to protect employees from the hazards associated with permit required confined space entries. This was most recently documented on February 1, 2017 at 9100 West Liberty Drive, Pleasant Valley, Missouri 64068. The employer is not protecting employees during the tank cleaning process when requiring employees to break the plane several times throughout the tank cleaning process for inspection purposes without first completing an entry permit.
