29 CFR 1910.134(c)(1): The respiratory protection program was not updated as necessary to reflect those changes in workplace conditions that affected respirator use: a) Throughout the facility, on or about 8/3/16: The site respiratory protection program had not been updated in 5 years. The lack of an updated program did not account for hazards including, but not limited to, manganese, copper and iron oxide overexposures. The program was deficient in the following was not documented: 1) Fit-testing procedures for air purifying half mask respirators, 2) updated respiration selection to include spray painters, 3) updating respirator selection procedures for welders, for whom respirators are provided, 4) procedures for inspecting supplied air for sandblasters, 5) filter changing schedule for DelMonox filtering system used with supplied air, 6) procedures for regularly evaluating the program including use of employee feedback
