29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about May 20, 2015, employees were exposed to struck-by, burn, and amputation hazards when the packaging machine used in the IQF2 (tray pack) room was not locked out each time the employee needed to reach inside of the machine to retrieve meat product/plastic packaging or to straighten out meat product/plastic packaging. An amputation took place on this machine on May 15, 2015. The employer has removed this machine from service. No abatement certification or documentation is required for this item.
