29 CFR 1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations: 1.) On or about April 15, 2015, the employer did not assure the permit required confined space program was implemented prior to employees entering the permit required confined space of the trans mat parts washer. The parts washer was not locked out during the procedure, exposing employees to caught in hazards from the chain conveyor, chemical hazards from chemicals such as but not limited to; Potassium Hydroxide and Phosphoric acid and water engulfment.
