29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: a. On or about March 3, 2015, at the roof top above the machine room, the catwalk platform to access the ice-maker machine controls was not guarded by a railing system, exposing employees to a 6 feet fall hazard.
