29 CFR 1910.119(g)(1)(i): The employer did not train each employee before being involved in operating a newly assigned process in the operating procedures as specified in paragraph (f) of this section. Jobsite: Tank Farm, including Reclamation Shed The employer did not adequately train employees in safe operating procedures. A newly assigned and inadequately trained employee mistakenly allowed the return of propellant to the wrong tank, thereby causing the tank to overflow and release highly flammable propellant to the atmosphere.
