29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: During the week of August 18, 2014, the employer failed to ensure group lockout is used when more than one employee is performing work on the Rolling Mill C0074. Employees enter the spinner shaft area to repair the chain sprocket on the spinner shafts with only one lock on the power supply.
