29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). Jobsite: The straight cut earth walls of a trench greater than six feet deep were not sloped, benched, or protected by a support or shield system exposing the employees to cave-in hazards. On or about June 09, 2014 a cave-in occurred injuring one employee.
