29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: a) On April 15, 2014, and times prior thereto, at the PACTRA warehouse in Midway, GA, the employer failed to ensure that a Hyundai PIT operator received recurrent training following a March 12, 2014 Dock 27 incident, exposing employees to numerous hazards including struck-by. ABATEMENT CERTIFICATION IS REQUIRED
