29 CFR 1910.23(a)(5): Every pit and trapdoor floor opening, infrequently used were not guarded by a floor opening cover of standard strength and construction. In the concrete elevator at Midwest Farmers Cooperative, Tecumseh, NE: The employer failed to ensure that the boot pit opening was guarded by a floor opening cover that was of standard strength and construction in that the cover had a triangular section that had been broken off measuring approximately 12 x 14 x 18 inches.
