29 CFR 1910.119(l)(1): The employer did not establish and implement written procedures to manage changes to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process: At the facility the employer had not implemented the management of change program to address changes to the Ammonia process; there has been several changes to the processes such as but not limited to the following: (a) for the removal of insulation from shut-off valve (6-L-407) on the 6-HTRL-5640-XR pipe line. (V-10002) (b) for the removal of insulation from valve (½-G-417) on an unidentified pipe line. (V-10002) (c) for the removal of insulation from valve (½-G-536) on an unidentified pipe line. (V-30001) (d) for the removal of insulation from valve (6-L-273) on the 6-HTRL-5613-XR pipe line. (e) for the removal of insulation from valve (¾-G-294) on unidentified pipe line. (V-10001) (f) for the removal of insulation from valve (6-L-271) on unidentified pipe line. (V-10001) (g) for the removal of insulation from the drain down valve (1-G-605) on unidentified pipe line. (V-30002) (h) for the removal of insulation from the liquid pump valve (1-L-612) on the 1-ODR-8005-XR pipe line. (V-30002) (i) for the removal of insulation from the liquid pump valve (4-L-623) on the 4-LLTRL-6754-XR piping. (V-30002) (j) for the removal of insulation from the 2-LTL-5806-XR low liquid ammonia piping between valves 2-G-606 AND 2-E-608. (V-30002) (k) the addition of a check valve and padlocked globe valve to the ammonia charge line inlet area. Employee(s) were exposed to release of ammonia vapor / liquid to the atmosphere that historically results in health hazards leading to injury and death to persons in the workplace.
