29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph. (a) 504 Lakeside Dr, Lee's Summit, MO 64064; A step ladder was set on the top level of the scaffold over the driveway and leaned against the exterior of the house. The scaffold was not secured from being pushed away from the house. Scaffold planks were not secured to the scaffold. Ladder legs were not secured to prevent them from slipping off scaffold planks. An employee was applying EIFS from the closed step ladder and was exposed to a potential fall hazard. Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
