29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) Facility, Shop Area: On or about November 20, 2013, employees who operated the Wiggins powered industrial truck (forklift) were not trained to safely operate this machine. (b) Facility, Whitney, PA: On or about November 5, 2013, the employee who operated the Caterpillar VC 600 forklift was not trained to safely operate this machine.
