29 CFR 1910.269(l)(2): The employer did not ensure that no employee(s) approached or took conductive objects closer to exposed energized parts than set forth in Tables R-6 through R-10: On or about September 15, 2013, at the job site at or near Entergy tower 197, north of Alpine, Arkansas, where employees were replacing insulators on shield wires above 500,000 volt transmission lines, the employees approached closer to energized parts than set forth in Table R-6, exposing employees to electrocution hazards in the following instances: a) The shield wire on the north side of the lattice tower b) The shield wire on the south side of the lattice tower
