29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) On or about August 22, 2013, an employee was exposed to an electrical hazard, while using a Simplex 100 ton jack, ID# 276211834-3, whose electrical cord outer jack was damaged (where the electrical cord connects to the jack), exposing the cords internal wiring. b) On or about August 22, 2013, an employee was exposed to an electrical hazard, while using a Simplex 100 ton jack, ID# 276211834-3, whose electrical cord had poor stain relief, where the electrical cord connects to the jack. c) On or about August 22, 2013, an employee was exposed to an electrical hazard, while using a Simplex 60 ton jack, ID# ECO 813-3, 69875, whose electrical cord outer jack was damaged (where the electrical cord connects to the jack), exposing the cords internal wiring. d) On or about August 22, 2013, an employee was exposed to an electrical hazard, while using a Simplex 60 ton jack, ID# ECO 813-3, 69875, whose electrical cord had poor stain relief, where the electrical cord connects to the jack. e) b) On or about August 22, 2013, an employee was exposed to an electrical hazard, while using a Simplex jack, ID# U43-2, whose electrical cord plug was missing the grounding pin.
