29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: (a) North Bay or Pull Through Bay: Employees were exposed to electrical hazards as the north light switch cover plate was broken and did not cover the whole electrical box. (b) Office: Outlet box adjacent to Front Door: Employees were exposed to electrical hazards as the outlet box that the Open sign is plugged into did not have a cover. (c) Middle office between two garage bays: Employees were exposed to electrical hazards as the outlet box that had a dorm sized refrigerator plugged into it did not have a faceplate. Abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19
