29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: An employee working in an approximately 3 foot wide, 52 inch deep, and 82 foot long trench, used an approximately 30 inch high step cut into the trench to enter and exit the trench.
