29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: Rig #4: On or about June 27, 2013, the employer did not ensure that the ground from equipment was permanent, continous, and effective for the following; a) centrifugal pumps, b) heat exchanger, c) toolpusher trailer, d) generator room, e) fluid pump motors, f) drilling rig. This condition exposed employees to the hazards associated with electrical shock, arc, sparks.
