29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors were not replace. On or about June 27, 2013, the employer did not replace damaged welding cables that have damaged insulation resulting in exposed conductors on the Miller welding machine located in the Main Building, exposing employees to electrical shock hazards.
