29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection. (a) Back side, new townhome construction located at 523 Micayne Circle, Smyrna, GA 30082: Employee was exposed to falls while installing cornice boards on a pump jack scaffold platform at heights of approximately 24-feet, 6-inches with a personal fall protection system which included a makeshift life-line, missing lanyard, missing rope grab and an anchor point with metal or similar connections.
