29 CFR 1910.269(l)(2): The employer did not ensure that no employee(s) approached or took conductive objects closer to exposed energized parts than set forth in Tables R-6 through R-10: At the AMEREN-UE Calloway Nuclear Plant, switchyard, specifically at Bus B, 52-4, an arc flash occurred on or about 04/02/13 severely burning employees. Employee(s) approached to within six inches of the live connectors located inside of this piece of equipment. Bus B, 52-4 was powered by 13.8kv at the time. Table R-6 requires that a minimum approach distance of 26 inches be maintained for that amount of voltage.
