29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grab rail, was not provided to assist the employees in mounting and dismounting the ladder: On or about January 24, 2013 - Job site at 45-11 Broadway Astoria, NY 11103 a) The employer permitted employees to use a job made ladder that was less than 3-feet above the point of access and could not be extended. It was not secured at the top and a grasping device was not provided. WRITTEN ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
