29 CFR 1910.147(f)(1)(v): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the equipment was not placed back into lockout prior to continuing the servicing or maintenance: a) On or about 12/14/2012, In the foundry: Employees performing maintenance work on the Inliner machine removed lock out devices from 800 power supply (hydraulic), raised the cylinder, and "spider" and then did not re apply locks prior to going underneath the raised "spider" and cylinder. Abatement documentation is required for this item.
