29 CFR 1910.119(f)(1): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and did not address the elements in paragraphs (f)(1)(i)-(iv): a) Old Acid Room Area - The employer did not develop or implement specific operating procedures for the Formaldehyde Loading System, on or about September 26, 2012. The current operating procedures are not specific to any system and do not contain the following elements: - Operating limits - Consequences of deviation - Steps required to correct or avoid deviation - Properties of, and hazards presented by, the chemicals used in the process, precautions necessary to prevent exposure, including engineering controls, administrative controls, and personal protective equipment - Any special or unique hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
