29 CFR 1926.502(i)(3): All floor hole covers were not secured when installed so as to prevent accidental displacement by wind, equipment, or employees: a) The employer placed loose (unsecured) pieces of plywood to cover where there were holes in the cement board floors. Employees were exposed to floor hole hazard if the plywood covers move during construction activities. Location: 60 Hoyt St, Brooklyn N.Y. On or about 7/31/2012 NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
