29 CFR 1910.178(q)(1): Power-operated industrial truck with defects were not taken out of service, and repairs were not made by authorized personnel as needed: On or about May 10, 2012 the brakes on the Kalmar forklift, S/N 561731A used to unload lumber and load cabinets, were not working. This condition exposed workers to the hazards of being struck by the forklift.
