29 CFR 1910.119(f)(3): The operating procedures were not reviewed as often as necessary to assure that they reflect current operating practice, including changes that result from changes in process chemicals, technology, and equipment, or changes to facilities: At the establishment, the operating procedures for ammonia process equipment were not reviewed and certified annually to assure that they reflect current and accurate operating practice. Employees were exposed to catastrophic release of ammonia which result in asphyxiation, explosion and flash fire leading to death and injury to persons in and near the facility. 29 CFR 1903.19(d)(1) requires certification and documentation that the abatement of the above violation is completed.
