29 CFR 1910.147(d)(4)(i): Lock out or tagout devices were not affixed to each energy isolating device by authorized employees: An employee performing maintenance on a packaging machine known as the "Ross" machine located in the Bone Room was exposed to amputation hazards. Lockout devices were not used to ensure de-energized of the air, water, and electrical sources. 29 CFR 1903.19(c)(2) abatement of the above violation was verified at the time of inspection.
