29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) was used, and at least annually thereafter: (a)Maintenance - Where employees were expected to approach the ammonia refrigeration system and stop the flow of liquid ammonia by manually closing the king valve and the employer provided full face respirators. The employer did not provide initial or followup fit testing, on or about July 21, 2011, Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
