29 CFR 1910.119(j)(2): The employer did not establish and implement written procedures to maintain the on-going integrity of process equipment. Mechanical integrity procedures were inadequate in that they did not spell out how often Inspection, Testing and Preventive Maintenance (ITPM) is to be done although some maintenance is getting done.
