Text For Citation: 01 Item/Group: 005 Hazard: LIFTING

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: (a)On February 16, 2010; in the yard, a Renfroe Model S Clamp (4 Ton 1 - 2") was used a move a steel plate 1u" thick. This clamp was not used in accordance with the instruction manual in that it was used for vertical lifts and turned into a horizontal lift. Among other methods, feasible and acceptable means to correct these hazards are to use a safer alternate method such as follow the requirements as outlined in: American Society of Mechanical Engineers ASME B30.20a-2001 Below-th-Hook Lifting Device: 20-1.4.4(b) The lifting device shall not be loaded in excess of its rated load or handle any load for which it is not designed. 20-1.4.4(c) The lifter shall be applied to the load in accordance with the instruction manual. Renfroe Model S Clamp Operation and Maintenance Manual: Application S, The model "S" is a vertical lifting clamp and is an effective tool for construction and erection. Operation S, Step 2, Select appropriate capacity and plate thickness. The model designation, capacity, and plate thickness are stenciled on each clamp. WARNING: Never exceed rated capacity or use on plates that are not within the range of plate thickness stenciled on the clamp. Lift only one plat on each lift. Always use a clamp with maximum plate thickness and rated capacity near equal to the thickness and weight of the plate being lifted. Operation S, Step 8, Commence lift. WARNING: The operator should position himself away from and fully clear of the member to be lifted. Do not commence lift until all personnel are clear of the area of the lift. Never stand under or near a member being lifted. Operating Aid, 6. DO Use clamps within their rated capacity. NEVER overload clamp. Certification, and documentation/evidence of abatement is required. Certification of abatement is required by 29 CFR Part 1903 and is to be submitted within ten (10) calendar days after the abatement date.ment
