SAVE: 1910.119(e)(3)(vi) The process hazard analysis did not address human facto rs: The employer does not ensure that the Process Hazard Analysis performed in Decem ber, 2009 addresses the following human factors: A.Items including but not limit ed to, vessels, pipes, valves, and heat exchangers, throughout the entire Platfo rmer area are not labeled. This could lead to equipment being improperly identif ied during maintenance and operations and therefore resulting in the release of chemicals, fire and/or explosion. In addition throughout the same area emergency exit signs and asbestos signs are not legible and/or visible. B.The employer do es not ensure that the Process Hazard Analysis performed on December 2009 addres ses the following human factor: Through employee interviews it was determined th at exposed employees are not provided sufficient time to perform a procedure to control an upset condition. Additionally, in the event of an emergency when an employee has to close valves, the PHA did not evaluate how this could be done safely. This exposed employees to possible fire and burn hazards in the ev ent of an accident or explosion. This violation was observed on or about March 3 , 2010. Pursuant to C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in com pliance with the standard, including addressing human factors during its PHA. On e method of abatement among others is to perform the human factors assessment du ring the PHA.
