29 CFR 1910.119(j)(6)(ii): Appropriate checks and inspections were not performed to assure that equipment was installed properly and consistent with design specifications and the manufacturer's instructions: On or about April 29, 2010, at WRB Refining, in the S-Zorb Unit the employer did not ensure that equipment was installed properly and consistent with design specifications: a.Misalignment of outlet piping coming from the reactor effluent filter was not installed correctly to prevent the escape of gasoline; b.The flange on the outlet piping coming from the effluent filter had a gasket installed that was not per original design specifications.
