29 CFR 1910.119(e)(3)(vi): The process hazard analysis did not address human factors: (a) Plant 81 - The PHA did not address human factors related to depressuring the Modules' high pressure loop during emergency conditions using the PRD bypass valve above the Cold High Pressure Separator. The inability to control the valve from a safer location than the top of the vessel was not considered.
