29 CFR 1926.501(b)(4)(ii):Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: On or about October 3rd, 2025, employees engaged in demolition operations located at 1701 West Pleasant Hill Road, Lincoln, Nebraska 68523, were exposed to fall hazards in that employees were walking between two holes that were uncovered and not protected from employees falling into. The holes were approximately 36"L x 36"W x 36" D.
