29 CFR 1910.179(h)(1)(ii): Sheaves carrying ropes which could be momentarily unloaded were not provided with close fitting guards or other suitable devices to guide the rope back into the groove when the load was applied again: Location - Dry Room - Pulley system round pulley parts and pulley system housing station. On or about 5/16/2025, round pulleys used to lift and lower rugs for drying were not prevented from falling out of the round pulley shaft, causing the potential for serious head injuries.
