29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. (a) Waiting room area left side of the building - On or about April 3, 2025, the employer exposed employees to a fall hazard, in that covers were not used for holes that had a 7' fall distance to a lower level.
