29 CFR 1910.179(l)(3)(i): Unsafe condition(s) disclosed during the frequent or periodic inspection(s) required by 29 CFR 1910.179(j) were not corrected before operation of the crane(s) was resumed: a) Sea Box Inc. - Legacy/East Fabrication Area: On or about January 22, 2025, employees were exposed to struck-by hazards while operating the #2 - 5 Ton Crane, Serial No. CH26168B. The periodic inspection records for Crane #2 indicated multiple safety risks/deficiencies, including but not limited to, hoisting wire rope was bent on both sides, sheave guards showed excessive wear and were improperly secured to the hook block and the hoisting limit switches were malfunctioning due to a bent and twisted upper limit mechanism. b) Sea Box Inc. - Legacy/East Jig Line Area : On or about January 22, 2025, employees were exposed to struck-by hazards while operating the #3 - 5 Ton Crane, Serial No. CH26169. The periodic inspection records for Crane #3 indicated multiple safety risks/deficiencies, including but not limited to, traversing gear oil level was low, hoisting wire rope had two bad kinks and had a flattened spot where it looked pinched and sheave guards were damaged on the hook block.
