29 CFR 1910.147(d)(4)(i):Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. (a) On or about December 19, 2024, and at times prior thereto, Employees had access to lathe Model # M-1950 Serial # 571808 Asset # 10501 without applying locks and/or tags to isolate the energy sources. (b) On or about December 19, 2024, and at times prior thereto, Employees had access to lathe Model# UM Serial # 7A Asset # 10500 without applying locks and/or tags to isolate the energy sources.
