29 CFR 1926.651(k)(1):Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. a) 1309 Crist Dr, Billings, MT, 59105: On or about September 20, 2024, and at times prior, employees were exposed to cave in hazards while locating a sewer line in an unprotected trench measuring approximately 11 feet deep with vertical walls. The employer failed to ensure that a daily inspection of the excavation was performed to identify hazardous conditions such as, but not limited to, excessive accumulations of water, sloughing and undermining of the sidewalks and asphalt road.
