29 CFR 1926.1211(b)(1): Each affected employee was not provided with the personal protective equipment (PPE) needed to conduct permit space rescues. On or about September 12, 2024, and at times prior thereto, employees in a lift station did not have a harness, egress extraction equipment or other readily available rescue equipment when coordination with emergency response was not made.
